Ordering GOS Forms- 17/06/2015

As you will be aware Primary Care Support Services have now relocated.

There is a new process for ordering GOS forms as follows:

• Access Contractor Supplies through the following link www.pcss.bsbc.nhs.uk/csordersstaffs
• Type postcode where prompted
• Type in a password (this will be used every time you log in)
• Follow prompts to order required forms
